WHAT A PATIENT CAN EXPECT FROM ALEGIS CARE

We appreciate the trust that patients show us by allowing us to care for them in their home. We respect our patients' privacy and try to communicate with them as much as possible to set expectations and confirm times.

This page provides a brief overview of our communication prior to an in-home visit and what should occur during that visit.

Here’s what you can expect during an in-home visit:

For Chronic Care Management (CCM):

  • Our doctor will become your primary care doctor.
  • Our doctor will do an assessment to understand your medical history, current medical conditions and medications.
  • We will develop a care plan to help you stay out of the hospital and stay in your home instead.
  • If you are hospitalized, our team will be there to check-up on you when you return home.
  • Our staff will assist you with scheduling your next or ongoing visits and answer any questions that you might have.

 For Complex Care Program (CCP):

  • Our health care professionals will support your doctor with in-home care for you.
  • We will work with your doctor to develop a care plan to help you stay out of the hospital and stay in your home instead.
  • If you are hospitalized, our team will be there to check-up on you when you return home.
  • Our staff will assist you with scheduling your next or ongoing visits in the home and answer any questions that you might have.

For Health Assessments:

  • A medical professional will complete the scheduled preventive screening or assessment
  • We will assist with scheduling another assessment, if needed
  • We will communicate the results of the screening to your doctor when completed
Prior to an in-home visit, Alegis Care will call to:
  • Let you know what to expect during the in-home visit
  • Tell you about how long the in-home visit will take
  • Verify your address
  • Verify the best contact number
  • Ask your Date of Birth for verification
  • Provide the name of the medical professional visiting you in your home
  • Schedule a date and time that works for you
  • Confirm your in-home visit the day before
